Fillable Form IT-2

Form IT-2 is used by taxpayers who file a New York State income tax return and have received one or more federal Forms W-2. It summarizes wage and tax withholding information for New York State, New York City, and Yonkers, and must be submitted with the taxpayer’s NYS income tax return.
